Information this website collects

This is a factual description of what the website actually does, written by the team that built it.

The callback form collects exactly four pieces of information, and no others:

  • Your full name
  • Your phone number
  • Whether you have Medicare (Yes, Medicare Advantage, Other insurance, or Not sure)
  • Your preferred time of day for a call

The form does not ask for, and provides no place to enter, your email address, date of birth, home address, member or policy number, medical conditions, medications, or any other clinical detail. There is no open text box on the form. This is a deliberate design decision intended to keep the amount of health-related information collected through this website to a minimum.

When you submit the form, the website also records the time of submission, the exact wording of the consent statement you agreed to, the page you first arrived on, any campaign parameters in the link you followed, your browser’s user-agent string, and a one-way cryptographic hash of your IP address. Your IP address itself is not stored.

How we use it

Information submitted through the form is used to contact you by phone or text message about remote patient monitoring services, as described in the consent statement on the form. It is not sold.

Tracking technologies
